After Hospital Care & Rehabilitation

After a hospital visit or accident, the most desirable place to be is back home. But sometimes you need extra help in your rehabilitation & recovery, which is where we can help.

Our after hospital care services include:

  • Therapy such as physiotherapy (mobility, strength, balance, etc) and podiatry (foot care)

  • Nursing support (wound care, etc)

  • Personal care

  • Injury & fall prevention

We can also help you with any required home modifications, which includes organising full occupational therapist guidance & reports, as well as the high quality building and construction works.

Major modifications include significant structural changes to a home such as lift and ramp installations and complex bathroom structures, including:

  • Initial home assessment

  • Occupational Therapy assessment

  • Arranging the preparation of building plans and specifications

  • Cost estimations

  • Project coordination of building modification

  • Post-modification assessment

Minor modifications include adding small, low-cost aids and equipment to the structure of a home to improve accessibility.

  • customised bed blocks

  • bed board

  • bath seats

  • chair platforms

  • hand-held showers

  • non-slip treatment

  • grab rails and hand rails

  • step modifications and step ramps

  • shower curtain rods and screen removal

  • door wedges

  • light switch and power point relocation

It's helped me so much. It's helped my shoulder, where I couldn't feed myself with one arm where I can now.
